Change to personal details

Name change

A name change does not result in a customer change (transcript); it is rather for example a change in name due to marriage or divorce.

The following details are obligatory:

  • Original surname and name of customer
  • New surname and name of customer
  • Number of consumption location
  • Address of consumption location
  • Contact details (telephone number, mobile number, email address)
  • Date of sending and signature (only if you are sending a request by post)

Attachments:

  • Copy of marriage certificate (change due to marriage)
  • Copy of birth-registry document about change in surname (other change of name)

Change to permanent address

The permanent address should correspond with your address stated on your citizen's ID card. If you will change your permanent or correspondence address due to relocation, you can directly report the change to both addresses, even if you currently have the former address in your citizen's ID card.

The following details are obligatory:

  • Surname and name of customer
  • Number of consumption location
  • Address of consumption location
  • Contact details (telephone number, mobile number, email address)
  • New permanent address
  • Date of sending and signature (only for written form)

Change to correspondence address

This is the address to which we will send you all mail. The address can differ from the address of permanent residence and consumption location.

The following details are obligatory:

  • Surname and name of customer
  • Number of consumption location
  • Address of consumption location
  • Contact details (telephone number, mobile number, email address)
  • New correspondence address
  • Date of sending and signature (only for written form)

Change to other details

The following details are obligatory:

  • Surname and name of customer
  • Number of consumption location
  • Address of consumption location
  • Contact details (telephone number, mobile number, email address)
  • Description of personal details that you would like to change
  • Date of sending and signature (only for written form)
